A hydrocele is an accumulation of fluid within the layers of the tunica vaginalis surrounding the testicle. Its types are generally classified according to their connection with the peritoneal cavity:
1. Communicating Hydrocele: This type of hydrocele is connected to the abdominal cavity due to a completely patent processus vaginalis. In this situation, intra-abdominal fluid can pass into the scrotum. The swelling becomes more prominent when the child is standing or active and decreases or disappears when lying down or resting. Although more common on the right side, it can often be bilateral.
2. Non-communicating Hydrocele: This is a hydrocele that has no connection with the peritoneal cavity due to the closure of the processus vaginalis. In this type, the swelling in the scrotum does not show a significant change throughout the day or with activity. It has different subtypes:
* Tunica Vaginalis Hydrocele (Testicular Hydrocele): The fluid is limited solely to the layers of the tunica vaginalis surrounding the testicle.
* Spermatic Cord Cyst (Cord Hydrocele): A cystic structure formed due to partial closure of the processus vaginalis between the internal inguinal ring and the scrotum.
3. Abdominoscrotal Hydrocele: A rare type of hydrocele. This large hydrocele extends from the scrotum into the abdominal cavity and can cause symptoms by compressing intra-abdominal organs and kidneys.
