In adults with type 1 diabetes, annual assessment of proteinuria and glomerular filtration rate should begin five years after the onset of diabetes. In those with type 2 diabetes, annual assessment should begin at the time of diagnosis. Patients should be referred to a nephrologist if there is progressive decline in renal function, detection of >300 mg/day proteinuria, or uncontrolled hypertension.
