Sudden and severe pain in the scrotum, scrotal swelling, abdominal pain, nausea and vomiting, abnormal positioning of the testicle, and frequent urination are symptoms that appear in testicular torsion.

Testicular torsion commonly manifests with the following symptoms:

* Sudden and severe pain in the scrotum (testicle)
* Scrotal swelling
* Visible redness and bruising of the scrotal skin
* Nausea and vomiting
* The anatomically rotated testicle is positioned higher than the other
* Frequent urination
* High fever
* Onset following scrotal trauma
* Increased pain when the individual elevates their genitalia (Prehn's sign)
* Intolerable pain during sleep in younger individuals

These symptoms are among the first observed.
