One of the primary technical challenges in endoscopic ear surgery is that the surgeon must manipulate the endoscope with one hand to secure the visual field while performing the surgical procedure with the other. This, however, generally does not pose a significant disadvantage for surgeons experienced in endoscopic ear surgery. Regarding visualization, while microscopic surgery offers a three-dimensional depth perception, endoscopic surgery provides a broad and clear field of view, though the image is two-dimensional. Nevertheless, the endoscope's maneuverability during the operation can provide the surgeon with a sensation of three-dimensional depth.
